Orchestra - Patron Donation & Season Ticket Form

We are very
grateful for the support we receive from Patrons and Season Ticket holders.
If you would like to support us in 2008 please supply the information
requested below. Gift Aid allows us to reclaim the tax you have paid on any
donation (but not on Season Ticket payments). If you are higher rate
tax-payer you can yourself claim the additional tax paid on the amount of
your donation, beyond the basic rate. If you have previously completed a Gift
Aid Form you do not need to fill in another, but you must notify us if you
are no longer paying tax, or fill in a new form if your name or address has
changed). |
